EXPLICIT CONSENT ON THE PROCESSING OF CUSTOMER PERSONAL DATA (INCLUDING PRIVATE PERSONAL DATA)
Within the framework of Disclosure on the Processing of Customer Personal Data and the Personal Data Protection and the Privacy Policy of TATMETAL Çelik Sanayi ve Ticaret A.Ş notified to me in accordance with the Law no. 6698 on the Protection of Personal Data (the “LPPD”) by TATMETAL Çelik Sanayi ve Ticaret A.Ş. (“TATMETAL Çelik Sanayi ve Ticaret A.Ş or the “Company”), I hereby AGREE AND ACCEPT to the following, except for Article 5/2, Article 6/3, Article 28 and other similar provisions of the LPPD;
- Collection, analysis and evaluation of information, preparation of proposals, policy approval, rejection, issuance, renewal, updating, including the spouses, children and the interests in the policy, conducting any research regarding claims history, insurance transactions, and performing assistance services,
- Obtaining expert opinion on health data while taking out a policy, conducting research regarding claims history, reinsurance and co-insurance processes
- Provision, review, evaluation, conclusion, approval, rejection, compensation and inspection of damage reports and management of damage processes
- Bringing recourse claims, execution and legal proceedings,
- Control and payment of provision/compensation provided by assistance firms,
- Preparation of reports within the scope of insurance activities
- Management of site audit and doubtful claims investigation processes,
- Sharing some data regarding the lawsuits and/or claims brought against me with the medical and actuary firms,
- Conducting corporate communication studies,
- Providing technical support from companies providing external services,
- Recording the incoming calls, using such records to improve the service quality,
- All kinds of surveys, including surveys which measure the quality of insurance services and customer satisfaction, and market surveys
- Sending all kinds of commercial electronic messages, conducting marketing operations including creating campaigns, and cross selling
- Creating personal opportunities for me related to products and services using all my personal data,
- The processing and use of my personal data including my private personal data (nationality, criminal conviction and security measures data, blood group, health data, religion, sect and philosophical belief, biometric and genetic data, etc.) for risk analysis and management limited to the purpose of the related processing, and the domestic and/or overseas transfer of such data to the parent company of the Company (including Regional Directorates and branches), Agents, Brokers, business partners, affiliates, public institutions, to companies rendering Health Expenditure Management services, to intermediaries, training firms, research firms, affiliated actuary companies, to the Medical consultant, assistance companies, experts, other insurance companies and reinsurance companies, companies rendering support services, to independent audit companies and to other third parties due to our various legal obligations; the storage of the same for the period of time required; and that I have been informed in this regard; and that I have read and understood this text, the Disclosure on the Processing of Customer Personal Data and the Personal Data Protection and Privacy Policy of TATMETAL Çelik Sanayi ve Ticaret A.Ş.
I hereby grant my explicit consent without any pressure in accordance with the provisions of the LPPD.
